    Superior Vision Plan members and their covered dependents can select from a wide choice of vision providers. The Superior Vision Network of over 40,000 providers nationwide consists of ophthalmologists, optometrists and opticians and optometric chain locations. Providers are located in every state and the network continues to expand to accommodate the needs of our valued clients and members.
  • Flexible Provider Selections:
    Superior Vision Plan members have the freedom to use different providers for their eye exam and their materials (eyewear or contact lenses). This allows the member to maximize their opportunities for product selection and optimize the value of their benefit.
  • Fast, Efficient Services:
    Our members have access to a national network of over 7,000 "one-hour" and "same-day" optical/optometric chain locations. In addition, Superior Vision providers can use their own in-house laboratory for lens fabrication or the services of a local lab to expedite delivery times and maximize quality assurances.
  • Paperless Systems:
    In-network providers take care of all claims and paperwork.
  • Inter-active Web Site:
    Members have access to Superior's interactive web site for benefits and provider listings at www.superiorvision.com.

    Competitive rates are available due to highly efficient administration and discounted fees negotiated with our providers.
  • Quality Assurance:
    Superior Vision's quality assurance systems are founded on National Committee for Quality Assurance (NCQA) standards.
  • Flexible Plan Designs:
    Plan designs offered under the Superior Vision Plan are unmatched in flexibility and design. For example, frame allowances are available from $75 to $175. Benefits are available both employer paid and employee paid (voluntary) and can be contracted as fully-insured or self-insured. Some restrictions apply. Benefits are offered with frequencies of 12 months or 24 months and can include various co-pay amounts. Plans include both in-network and out-of-network service opportunities.
  • A Wellness Program:
    Comprehensive eye examinations performed by one of the Superior Vision Plan member ophthalmologists or optometrists can lead to early diagnosis and appropriate treatment of eye diseases and other bodily disorders. Regular eye exams can save employees from more serious health problems, reduce absenteeism, improve general wellness and maintain peak performance.

Additional Discount Features

  • Refractive Surgery Discounts:
    Superior Vision has contracted with over 600 ophthalmic surgeons nationwide to provide substantial discounts on refractive procedures, such as LASIK. These procedures are performed through more than 500 conveniently located refractive surgery centers.
  • Cosmetic Eye Lid Surgery Discounts:
    Similar to the refractive surgery discount feature, specially contracted Superior Vision network surgeons provide discounts to Superior Vision members on the procedure of blepharoplasty (cosmetic eye lid surgery).
