Flexible Plan Designs
The Superior Vision Plan provides a number of options to satisfy the needs of the payor and the plan member. Plan designs are flexible to meet the needs of our clients.
Plan Eligibility
The minimum number of covered employees for employer paid plans is 10. For voluntary plans we require a minimum of 10 enrolled employees.
Coverage Overview
Superior Vision Plan benefits provide for fully covered eye examinations plus eyeglasses and contact lenses.
Lens Benefits
Covered eyeglass lenses are standard glass or plastic (CR39) in clear or rose tint #1 or #2. Frames are available at five different levels of retail value. Contact lenses are available through allowances of $100, $120, $150, or $200 per benefit period.
Frame Benefits
The following are the categories of frame values that are offered. Members can select any frame in the providers inventory up to the retail allowance without out-of-pocket expense. For frames priced higher than the allowance, the member pays the difference in price directly to the provider.

Frequencies of Benefits
The Superior Vision Plan offers insured benefits under a variety of schedules. These are Select, Preferred and Premium. The key difference in these schedules is the interval when services are available. The following outlines the timetable for benefits under each schedule. Other variations of frequencies are available.

Special Note: Plans can be written as "materials only", excluding the eye exam, or "Exam Only", excluding the materials.
In-Network and Non-Network Services
Benefits are available from both in-network and non-network providers. The reimbursement levels for non-network benefits will vary by state.
