Our goal at Superior Vision is to make using your benefits easy to understand and easy to use. Maintaining healthy eyes is important in maintaining a
healthy body. Below is a quick overview to using your benefits. Benefits vary by employer; to access your specific benefits, including co-pays, allowances,
frequency of use, and usage history, log in to the secure section of the member portal and go to Benefits Coverage. Click on “Need Help Getting Started?”
in the navigation bar for more information on accessing the secure member portal.
In-Network and Out-of-Network Providers
Your vision benefits are offered through a PPO (Preferred Provider Organization) plan. We have “in-network” providers (those for whom we have a PPO contract)
and “out-of-network” providers (no PPO contract). This means that you can obtain products or services through any provider you choose, though you’ll generally
pay less with our in-network providers. When visiting an in-network provider, you are responsible for paying any applicable co-pay and for items that are not
covered, or that exceed your benefit limitations. When visiting out-of-network providers, you pay for all services in full, and then file a claim with Superior
Vision for reimbursement according to your out-of-network benefits schedule.
If you have questions about the amount the provider is asking you to pay:
- Remember to identify yourself or your dependent as a Superior Vision insured member.
- Request that the in-network provider call Superior Vision’s Customer Service department before you pay in full.
Our Strong Provider Network
We are proud of our network as we have one of the largest, most diverse nationwide. This is important to you as it gives you
the freedom of choice - you may see an ophthalmologists, optometrists, or opticians and optical chains eye care provider.
You can make your choice based on your daily work schedules, budget, shopping patterns, and healthcare system preferences.
If your provider of choice is not currently contracted with us, you may fill out a Provider Nomination form and our Provider
Relations team will contact the provider to encourage their participation.
Discount Plans Overview
Many of our providers will accept discounts. You can find out which of our providers accept discounts by looking at Locate a
Provider on our website or by calling Customer Service. Some discounts vary by provider but are the best possible discounts
available to Superior Vision members. For information on LASIK discounts, click on LASIK Info in the navigation bar.
SVP 8-20 Discount Plan
This discount applies to your covered pair of glasses. On your covered frame, you receive a 20% discount off the difference
between the covered retail frame allowance and the retail price of the selected frame. Discounts do not apply when prohibited
by the manufacturer. Upgrades and options to your covered lens will be discounted by 20% off the retail rate. There are some
options on certain lens types that are discounted to a specific amount and this list does not include high end, brand, or non-standard
options. On all standard lenses, factory scratch coat is $13, UV is $15, standard anti-reflective is $50, glass coloring is $35, and
solid and gradient tints are $25. On standard single vision lenses, standard high index 1.6 is $55, polycarbonate is $40, and standard
photochromic is $80.
Material Discounts on Additional Purchases
Prescription eyeglass lenses and frames are at 30% off retail. Lens options and upgrades are at 20% off retail. Contact lenses are 20%
off retail except for disposables, which are 10% off retail.
