Refractive Surgery Discount Feature
Superior Vision has contracted a network of over 600 refractive surgeons nationwide who specialize in the popular elective procedures of radial keratotomy (RK), photo-refractive keratectomy (PRK), conductive keratoplasty (CK), and LASIK. These providers offer members a 20% discount off their usual and customary surgical fees for these procedures. In addition, we have contracted with LasikPLUS for additional discounts in certain states. Their providers offer 15% discount off the professional surgical charge, as well as 15% off the facility fee.
There are no claims filed, no authorization by Superior Vision, and no co-payments required. The discounts are taken at the time of you visit, and directly between you and our in-network provider. Look for the "RF" coded next to the providers name listed in our directory for in-network providers participating in this special discount feature.
All "RF" ophthalmologists undergo detailed credentialing requirements in accordance with NCQA national standards prior to acceptance. If you would like to nominate a provider for participation in our Refractive Surgery Network, please complete a Provider Nomination Form and return it to us. One of our Provider Relations Managers will be happy to follow-up.
Remember: Discounts are available only from Superior Vision Plan in-network providers who are identified in the provider directory with a "RF" as part of the services they provide under the Plan.
