Provider nomination requests may be made by calling the Superior Vision Services
Provider Relations or Member Services Departments or you may obtain a Provider Nomination
Form through your employer. Every effort will be made to consider your nomination.
However, qualifying and credentialing guidelines may restrict provider participation
in some geographic areas.
Superior Vision Services makes every attempt to assure that the information regarding
our provider network is accurate as of the date of your inquiry. As a provider�s
contractual status with Superior Vision Services may occasionally change, Superior
Vision Services recommends that the member call our Member Services or Provider
Relations Departments to verify a specific provider�s current participation status
in the network.
