Superior Vision is taking action to assist members with accessing replacement eyewear if they have been affected by Hurricane Harvey and are located in one of the Texas Counties listed on FEMA’s Major Disaster Declaration (DR-4332).

Group Plans

If you have your vision benefit through your employer, the following services are available:

  • Replacement eyewear: Please call us for assistance with replacement eyewear if your glasses or contact lenses have been lost or damaged during Hurricane Harvey.

  • Assistance in Finding an Eye Care Provider: If you need to find an in-network provider, call our Customer Service Center at the number below or use our online search tool.

Health Plans

If you have your vision benefit through a health plan program such as Medicare Advantage or Medicaid, the following benefits and services are available:

  • Replacement eyewear: Please call us for assistance with replacement eyewear if your glasses or contact lenses have been lost or damaged during Hurricane Harvey. Some plans have replacement eyewear benefits. If your plan does not, contact us and we can assist you.
